Culture is SOFT stuff
for sissies! We don't
do that here
Oh no! Not
another survey!
Nothing changes
anyway

People's principal values and beliefs are the strongest driver for people's attitude.

And people's attitudes drive behaviour. Safe or Unsafe.

Influencing people's beliefs and attitude in safety is the most efficient way of improving safety.

to recognize the limit of procedures, training and punishments in advancing safety.

Do you know what your staff's values and beliefs are with respect to safety?
Is your staff following rules because they have to or because they want to?
How do you control the safety related behaviour of your staff?
How do you make sure that your people act safely?
Do you want your people to act safely?
Are your managers leading by example?
How do you demonstrate that you are committed to safety?
What are your actions to create a positive safety climate, where required perceptions and attitudes towards safety are clear? And opinions are freely exchanged?
How do your people know what behaviour is expected from them?
How do you make sure that mistakes are not happening again?
Are you aware what the staff attitude towards safety is?
Do you know what your people are doing when you are not looking?
Have you defined the mission of the organisation such that all people know what the intent of their behaviour must be?
Are your people wanting to prevent safety events?
Are your people ready to intervene when things do not go as planned, to keep it all safe? (resilience)
Is the staff trusting that the one working next to him is safe as well?
Can staff involved in an incident, in any sort of way be set back, blamed, punished, or even sacked?
Is exemplary staff behaviour encouraged, stimulated, incentivized, rewarded?
Do your people want to improve safety levels?
Are people freely speaking about unsafe behaviour and report about it, even if they were themselves involved?
Have you created an improvising, flexible hierarchy in case of mishaps or crisis?
Are you expecting the next incident?
Do you have a risk register?
Do you have examples of taking unnecessary risks?
questions cover all levels of the pyramid and are meant to provoke discussion and thought ultimately leading to the recognition of the importance of safety culture.
