Hey, I'm Allison Mullins and I'm going to talk about childhood at proxy.
- 00:07 - 00:11
So just as a review, childhood of praxis speech is a speech sound disorder.
- 00:11 - 00:15
Um It's pretty difficult to differentially diagnose it,
- 00:15 - 00:19
especially from a phonological disorder or articulation disorder.
- 00:19 - 00:20
Um
- 00:20 - 00:21
It
- 00:21 - 00:24
is an issue with motor planning and
- 00:24 - 00:26
programming as well as sequencing speech sounds.
- 00:27 - 00:31
And it presents as arid speech sounds and prosperity errors.
- 00:32 - 00:35
So I feel like this visual kind of summarizes
- 00:35 - 00:38
approximate pretty well for the most part kids that
- 00:38 - 00:40
do have childhood a proxy of speech want to
- 00:40 - 00:42
say something and they know what they want to say
- 00:42 - 00:45
but when it comes to getting the
- 00:45 - 00:48
articulator to cooperate,
- 00:48 - 00:50
that is another
- 00:50 - 00:55
um story. So something goes wrong with the motor plan or motor program.
- 00:56 - 00:57
So
- 00:58 - 00:58
some,
- 00:58 - 01:02
let's talk about some of the more characteristics and I'll go kind of briefly
- 01:02 - 01:05
on these because I know we're looking at it with the ei population.
- 01:05 - 01:06
So with ASHA
- 01:07 - 01:10
says it's been big consensus
- 01:11 - 01:13
four Childhood a proxy of speech. There's three kinds
- 01:14 - 01:15
characteristics. We look for
- 01:16 - 01:18
inconsistent errors in both continents
- 01:18 - 01:19
and vowels
- 01:19 - 01:24
in repeated um productions of syllables or words
- 01:24 - 01:29
inappropriate property as well as um lengthened or disrupted
- 01:29 - 01:30
like co articulation.
- 01:32 - 01:35
So I won't spend too much time on this either.
- 01:35 - 01:37
But some other characteristics that you might know
- 01:37 - 01:39
are groping or searching for a sound.
- 01:39 - 01:42
The longer a word or sentence gets,
- 01:42 - 01:45
the more difficult it is for a child with a proxy of speech
- 01:45 - 01:48
a slower rate invoicing errors so they tend
- 01:48 - 01:51
to voice um voiceless continents and visual distortions.
- 01:53 - 01:57
So also show insertions and patterns that might
- 01:57 - 01:59
appear phonological but can be meteoric in nature,
- 01:59 - 02:00
such as
- 02:00 - 02:02
constant cluster reduction
- 02:03 - 02:05
but really focusing in on the early
- 02:06 - 02:08
intervention population or the
- 02:09 - 02:09
like,
- 02:10 - 02:14
you know, 1234 preschool age range.
- 02:14 - 02:19
Um What this is going to present itself as is delayed first words
- 02:20 - 02:24
les babbling than um we would expect for an infant
- 02:25 - 02:26
or um
- 02:27 - 02:27
young child
- 02:28 - 02:30
a smaller phonetic inventory.
- 02:30 - 02:34
So we may not see some of those earlier developing consonants
- 02:34 - 02:38
um in their repertoire. We also may see like my bowels
- 02:39 - 02:43
um that the words that they do produce may be difficult to understand.
- 02:43 - 02:48
And there's also reports of ghost words with a proxy of speech where parents
- 02:48 - 02:50
will hear a child say a word and then never hear it again.
- 02:51 - 02:52
And
- 02:52 - 02:53
so
- 02:53 - 02:54
note
- 02:54 - 02:59
that it is, we may see these things, but as we all know, these are also very,
- 02:59 - 03:02
very similar things to what we see in speech delay
- 03:02 - 03:05
or language delay as well as many other syndromes.
- 03:05 - 03:05
So,
- 03:06 - 03:11
a proxy is very difficult to differentially diagnosed until the child gets more.
- 03:11 - 03:15
Language between two and four are really is really the
- 03:15 - 03:17
earliest they diagnosed it and that is still pretty early.
- 03:18 - 03:19
So
- 03:20 - 03:25
uh it can occur with feeding difficulties, oral and limb, approx,
- 03:25 - 03:27
CIA gross and fine motor delays,
- 03:27 - 03:31
sensory issues as well as lack of motor coordination.
- 03:31 - 03:32
So those can occur.
- 03:33 - 03:34
And
- 03:35 - 03:38
so causes can be congenital
- 03:38 - 03:41
from birth or required. Um
- 03:41 - 03:47
Just like many other speech sound disorders. We don't fully understand a lot of um
- 03:47 - 03:48
a lot of the cause.
- 03:49 - 03:53
You can also co occur with disorders and syndromes like um
- 03:53 - 03:59
autism will will Prader, Willi excuse me, fragile x.
- 03:59 - 04:02
Um those are just some of the comments syndrome
- 04:03 - 04:05
um also other neurological causes.
- 04:05 - 04:08
And they are doing some really interesting research on genetically.
- 04:11 - 04:11
So
- 04:13 - 04:16
let's see how is it going to affect early language.
- 04:16 - 04:19
Well, like I mentioned earlier, we're going to see delayed language development,
- 04:19 - 04:21
especially in expressive language.
- 04:21 - 04:24
The child may have no words when we expect. Um
- 04:24 - 04:27
at least a few, especially at 12-18 month range.
- 04:27 - 04:29
Um They are at risk,
- 04:29 - 04:33
increased risk for weaknesses and phonological foundations and literacy skills.
- 04:33 - 04:38
And it may also present a problem with pragmatics like a child may be avoiding um
- 04:39 - 04:41
avoiding interaction because
- 04:42 - 04:44
quite frankly it's difficult to communicate
- 04:45 - 04:47
trans disciplinary services the child may receive.
- 04:47 - 04:50
Um depending on one of our co occurrences they may have.
- 04:50 - 04:52
In addition to speech language therapy,
- 04:53 - 04:56
they may have physical therapy to address clumsiness and gross motor skills,
- 04:57 - 05:01
occupational therapy to address fine motor skills and or feeding
- 05:01 - 05:04
and then maybe some physicians and other team members for
- 05:05 - 05:07
syndromes and other medical conditions.
- 05:09 - 05:10
Some resources.
- 05:10 - 05:15
Um ASHA has a great parent friendly information page where no jargon.
- 05:15 - 05:17
It's nice and um played out well
- 05:17 - 05:19
a proxy a kids is a good
- 05:20 - 05:24
organization that speech language pathologists and families can join.
- 05:24 - 05:28
Um Mayo clinic has some good jargon free
- 05:29 - 05:30
detailed information as well
- 05:30 - 05:34
for clinicians. The ASHA practice portal is a wealth of information as always.
- 05:34 - 05:38
And this last resource is a free childhood of
- 05:38 - 05:41
proxy A speech training from DR Eva Strand,
- 05:41 - 05:46
who is considered one of the if not the um kind of leading expert in the field.
- 05:46 - 05:51
She works at the Mayo Clinic and has done a lot of work into this topic,
- 05:51 - 05:53
but it's a free one and you can get to you,
- 05:54 - 05:58
which is exciting. Um And here are my references.
- 05:59 - 06:03
So that is childhood approximate speech. I hope you enjoyed my presentation
- 06:03 - 06:04
and thanks.
