PCAC Committee meets to:

Preceptor Tax Credit Process Flow

Admin Committee sends:

  • N358 forms ready for signature to DOH
  • File with preceptor contact names, emails, mailing address and number of credits to DOH and DoTAX
  • Report to Governor, Lt Governor, DoTAX, DOH, State Legislature, and on website

Draft Process for Review 9 28 2018

1) Certify credits per preceptor and complete documentation of Form N-358 per taxpayer to be submitted to DOH (who distributes forms to taxpayers), and

3)Finalize annual report of preceptor activities

Provider agrees to precept students with 1+ eligible programs

  • N-358 forms by email or USPS to recipients
  • Message to any individual who did not receive form with reason (ineligible, maxed out on budget, etc)

One time only: Submits unique identifying information (name, demos, practice type, etc) AND attestation of no compensation for precepting. May be updated anytime.

1) Tracks preceptor activity by individual (no more than 5 credits/preceptor in annual year);

2)Tracks total preceptor activities eligible (no more than 1,500 activities/$1.5M in annual year);

3) Posts number of credits allocated throughout year; and

3)Notifies DoTAX, DOH and posts on website if max is met prior to end of calendar year.
