Neurogenic Bladder

Definition

Spastic (overactive)

  • A disorder of all bladder dysfunction due to disease of the central nervous and peripheral system thats involved to control micturition (urination) process.

  • Three types:
  • Spastic
  • Flacid
  • Mixed

Clinical Manifestations

  • affects the T12 vertebrae (upper motor neuron lesion)
  • bladder is overactive and person cant control
  • feel of sudden urge to urinate
  • fails to store urine properly.

  • weak or dribbling urinary system
  • inability to urinate or straining to urinate
  • Polyuria
  • painful urination (may lead to urinary tract infection)
  • urgency urinating

Wein and Dmaochowski (2011)

  • Spinal cord injury

Pathophysiology

Upper motor neuron lesion (T12 and above)

(affects spinal reflex that controls micturition process)

Route: Per Oral

Dosage: Adults 5mg

Not suitable for children

Generic name: Oxybutynin

Route: Per Oral

Dosage: Adults 5mg, bid/tid/ 20mg max, Elderly initial dose 2.5mg bid and then increase prn. Children 5yrs + 5mg bid/15mg max.
