Telephone Triage of the Cardiac Patient

To develop a streamlined triage template for patients complaining of chest pain
To create a template that all triage nurses will use equally
To decrease the amount of free text that is put into a triage encounter
Reduce the amount of unnecessary ER and Immediate care visits

Increasing variability in triage documentation by RNs (H. Wimsatt, personal communication, January 14, 2014)
61% of adults either did not need immediate treatment or were treatable in a clinic or doctor's office (Billings, 2013)
Centers for Medicare & Medicaid Services working on new initiatives to decrease inappropriate use of Emergency Departments (Mann, 2014, p. 2.)
Description of intervention
Determined needs for template based on input from staff RNs
Completed a list of pros/cons about current triage protocol
Used current Chest Pain protocol to develop needed fields in triage template
Created improved electronic triage template for adult patients suffering chest pain

Patients stated increased satisfaction of care
Felt appointments were faster
Stated relief of not needing ER visit
Statement of local ER RN decrease in visits from Crusader Community Health patients, 1-2 chest pain patients per month, over the course of 3 months. (D. Girone, personal communication, April 20, 2014)
Agency evaluation
RNs stated increased confidence of triaging chest pain patients.
RNs were able to describe key symptoms that triggered ER visits in patients suffering chest pain.
RNs were able to describe the difference in symptoms of patients needing ER, same day appointment and next day appointment
Medical unit manager stated uniformity of charting among nurses for telephone triage patients
