  1. Hi, everyone. My name is Kyle Lang.
  2. I'm a bachelor of kinesiology student here at the University of Vagina,
  3. majoring in human kinetics.
  4. This semester my full replacement was at the past the hospital on
  5. the Accountable Care Unit on for a Under Dr Hannah Buhari Wall
  6. background information regarding the Accountable Care Unit.
  7. It was designed in the United States by Dr Jason
  8. Stein and replicated here in Regina by Dr Ron Taylor.
  9. It offers a team based approach to patient care and focusing on increasing patient
  10. and staff satisfaction while decreasing length of
  11. stay readmission rates and decreasing mortality.
  12. As a kinesiology student on the accountable care unit,
  13. your day to day tasks result in working on
  14. your special project and the standardised data collection.
  15. Q. I projects on the A. C.
  16. You are very heavily emphasis to replicate in order to gain support from the
  17. S h A and to replicate the A c you provincially across Saskatchewan.
  18. My project was given to me due to my one dimensional experience as a paramedic and
  19. to be able to provide insight for to those who are on the accountable care unit,
  20. my special project is increasing communication during
  21. inpatient to outpatient transitions of care.
  22. There's been a recognition that seven paramount pieces of information should be
  23. community communicating with the patient upon
  24. discharge to reduce unnecessary rehospitalization.
  25. And there is identification that there was
  26. a co dependency between the physician and
  27. nurse to relay necessary information when it
  28. wasn't actually being given to the patient.
  29. My auditing processes done daily when I audit discharge charts from
  30. the physician and identify if in which a physician and nurse district
  31. she was filled out identified all seven pieces of information was
  32. given on the discharge sheet from the physician and the nurse,
  33. provide them a score and grade it and implement that, said data into Excel.
  34. The said data is then shown at weekly touch points with staff,
  35. physicians and other colleagues.
  36. Here is an example of some of the data I've implemented into Excel
  37. and as well as some of the grass I've
  38. created for meetings weekly and monthly with other staff
  39. as well as Here's My Timeline and Milestone
  40. try to created for the accountable Care unit.
  41. I'm currently still working on initial data collection from the by
  42. the student and as well as it will be carried on
  43. by the next student due to the amount of data that
  44. they want to be able to provide to the S A.
  45. J.
  46. The value of my project is to create
  47. a universal physician and nurse discharge sheet,
  48. therefore, to further implement the A.
  49. C. U S values of reducing costs and unnecessary readmissions.
  50. And
  51. as you can see, this is the nurse discharge information sheet.
  52. It is very broad and often provides information that is non pertinent or helpful,
  53. as well as the current physician hospitalist to start sheet,
  54. which is very broad and often is very minimally filled out and unhelpful.
  55. For the patient opposed to Dr Hannah Buhari Wallace,
  56. which is very specific and provides very specific amounts
  57. of information that is often filled out by the docks
  58. who do decide to use it patients then bring
  59. it to their follow up appointments with their doctors.
  60. My learning objectives this semester will gain
  61. was gain increased confidence with Microsoft Excel,
  62. which has been done due to data implementation into excel
  63. time reduction while auditing,
  64. which is now reduced from an hour to 45 minutes to audit 10 charts,
  65. and as well as increased presentation, experience and efficiency,
  66. which is done due to weekly touch point meetings.
  67. Thank you very much for listening
