  1. Hi and thank you so much for joining me today
  2. for a short audio tour of our project entitled left
  3. ventricular hypertrophy and biomarkers of cardiac damage and stress and
  4. aortic stenosis.
  5. My name is Elliot Stein and I am a second
  6. year internal medicine resident at Vanderbilt.
  7. I'm really excited to apply for fellowship in cardiology This
  8. coming summer I've had the distinct honor of working with
  9. my P.
  10. I. Dr brian lemon and the rest of this fabulous
  11. cadre of co authors on this project.
  12. We studied the relationship between left and curricular hypertrophy, cardiac
  13. damage and stress and the probability of survival at five
  14. years after Tavern.
  15. So we'll start with a little background.
  16. Left ventricular hypertrophy often develops in response to pressure overload
  17. from aortic stenosis and it is already well known that
  18. there is an adverse association between greater LVE me left
  19. ventricular mass index and worse clinical outcomes among patients with
  20. severe as treated with Tavern.
  21. But what is not yet known is whether preoperative biomarkers
  22. of cardiac damage and stress namely plasma troponin and anti
  23. pro BMP can further risk stratify those with severe A.
  24. S. And left ventricular hypertrophy.
  25. Going on to receive Tavern.
  26. We used a multi center prospective registry of patients with
  27. severe A.
  28. S. Undergoing Taber which yielded 674 subjects with a mean
  29. age of 82 years and was approximately 44%.
  30. Female Patients had a mean sts score of 5.3.
  31. We use core lab measured LV mass index.
  32. In addition to other standard echocardiogram thick dimensions, We also
  33. obtained preoperative plasma troponin Anti Pro Bmp on bank specimens
  34. which were all quantitative ana rocha say we then used
  35. adjusted cox proportional hazard models and restricted cube explains to
  36. evaluate associations between L.
  37. V. H.
  38. And biomarkers and all cause death.
  39. We made adjustments for age sex Bme sts, core hemoglobin,
  40. creating clearance Taber approach left ventricular ejection fraction, presence of
  41. oxygen dependent lung disease, mitral regurgitation severity, presence of a
  42. fib or flutter and presence of active cancer.
  43. So now let's take a look at figure one.
  44. This figure shows the proportion of biomarker elevation among females,
  45. males and both.
  46. And it also stratified biomarker levels by L.
  47. V. H.
  48. Severity. Looking for some troponin, which is represented by the
  49. blue graphs.
  50. You can see that in females, males in both sexes
  51. together, there is a statistically significant up trend in the
  52. proportion of patients with elevated troponin.
  53. Moreover, absolute troponin concentration increases as L.
  54. V. H severity increases, which is represented in the box
  55. and whisker plots now moving over to anti pro Bmp,
  56. represented by the green group graphs.
  57. We can observe the same findings hold true as with
  58. your opponent.
  59. Finally, when combining troponin and anti pro Bmp as expected,
  60. increasing L.
  61. V. H.
  62. Results in a statistically significant increase in the fraction of
  63. patients who had elevations in both biomarkers.
  64. So we can conclude from this figure that elevations in
  65. these biomarkers are both common and they are also correlated
  66. with worsening L.
  67. V. H.
  68. Now I'd like to return our attention to the table.
  69. This table describes each of the four adjusted cox proportional
  70. hazard models that we studied in the first model.
  71. We compared moderate severe LDH to know and mild L.
  72. V. H.
  73. We found that the five year mortality rate for moderate
  74. severe LDH trended higher than with no mild L.
  75. V. H.
  76. With a P value of 0.059.
  77. However, this effect was attenuated after adjustment as you can
  78. see in model one of the table for context.
  79. Partner trial data showed an independent association between increased LDH
  80. and five year mortality with a similar point estimate as
  81. our study.
  82. But the partner trial had higher power with a sample
  83. size around 4000 patients moving to model to.
  84. We compared moderate severe LV.
  85. H. As sub classified by biomarker elevations to know mild
  86. and LDH.
  87. Which serves as the reference.
  88. You can see here that only when patients had both
  89. moderate severe LV H and have an elevation in troponin
  90. or anti pro BNP, is there a statistically significant hazard
  91. of all cause death at five years?
  92. 1.4 fold for troponin and 1.3 fold for anti pro
  93. Bmp. Since we observed a high percentage of patients with
  94. no mild L.
  95. V. H.
  96. Having elevated biomarkers.
  97. As I showed in figure one, we also asked whether
  98. biomarkers could risk stratify patients with no mild LDH, which
  99. is shown in Model three.
  100. Here we separate the no mild and moderate severe LV.
  101. H groups into those with elevated biomarkers versus those with
  102. normal biomarkers.
  103. And then compare them to the reference of no mild
  104. L. V.
  105. H. Without biomarker elevation.
  106. Here we observed that even among those without significant L.
  107. V. H.
  108. There is an excess hazard of death on the order
  109. of 1.78 fold for troponin And 1.37 fold for anti
  110. pro bmp.
  111. Expectedly this result holds true and the relationship is even
  112. stronger among those who have both moderate severe LV.
  113. H. And biomarker elevation.
  114. You can see that the elevated hazard that the hazard
  115. of all cause death is 2.2 for troponin elevation and
  116. 1.7 for anti pro BMP elevation, importantly, the deleterious effect
  117. of L.
  118. V. H was completely attenuated when comparing those who had
  119. modern severe LV.
  120. H. Without a biomarker elevation to the reference, suggesting that
  121. the excess risk is captured by biomarkers.
  122. Finally, model for Model four looks at the hazard of
  123. all cause death among those with and without biomarker elevations.
  124. When controlling for L.
  125. V. H.
  126. We observe that the hazard of all cause death as
  127. 1.94 and 1.45 for for cardiac troponin, anti pro BMP
  128. elevation respectively.
  129. Furthermore, there is no interaction between troponin and anti pro
  130. BMP, suggesting that biomarker elevations are associated with all cause
  131. death independent of L.
  132. V. H.
  133. And independent of each other.
  134. To conclude this point, the bottom of the table says,
  135. is the interaction between L.
  136. V. H.
  137. Severity and biomarker concentration, finding that there is no interaction
  138. between these variables, suggesting again that the excess risk seen
  139. in those with biomarker elevations is independent of LDH severity.
  140. As you can see, there are several more figures.
  141. But in the interest of time, I will not go
  142. over them.
  143. During this brief tour, I'm of course happy to answer
  144. any questions about them during the chat.
  145. So to summarize the bottom line is this, we used
  146. a multi center prospective registry yielding 670 for patients with
  147. severe A.
  148. S. Who received Tavern.
  149. We showed several key things.
  150. First, the majority of subjects with severe A.
  151. S. And her cohort had elevations in plasma levels of
  152. cardiac troponin and anti pro BMP.
  153. These elevations became more common as the amount of left
  154. ventricular hypertrophy increased.
  155. Second, the biomarkers cardiac troponin and anti pro Bmp were
  156. able to robustly risk stratify patients with moderate severe LV.
  157. H. Undergoing tavern, but also risk stratified patients with known
  158. mild LVMH.
  159. In other words, the presence of L.
  160. V. H.
  161. Did not alter the increased hazard of death conferred by
  162. elevated cardiac troponin and anti pro BMP.
  163. When examined together, troponin and anti pro BMP reached independently
  164. associated with mortality after tavern.
  165. Whereas L.
  166. V. H.
  167. Was not, I'll finish with a few points of discussion
  168. First, the limitations one our sample size was likely too
  169. small to show that L.
  170. V. H.
  171. Severity is related to all cause death to.
  172. We did not have follow up echocardiogram phycor biomarker data
  173. to assess for regression Three, we could not report rehospitalization
  174. rates among each group because that data was not available
  175. for our cohort was unfortunately highly biased towards white individuals
  176. and five all patients in the study had symptomatic s.
  177. Nevertheless, to our knowledge, this is the largest study incorporating
  178. core lab red echo and centrally measured biomarker concentrations.
  179. To date to conclude the increased hazard of mortality associated
  180. with greater LDH observed in prior studies appears to be
  181. due at least in part two more frequent and severe
  182. cardiac damage and stress, rather than increased LV mass per
  183. se. These findings may have important implications for risk stratification
  184. and treatment of patients with S.
  185. Thank you so much for joining me.
