00:01 - 00:06
Hi my name is Victoria. Um so today I'll be doing my clinical presentation for 4:34.
00:07 - 00:11
And we're going to get started with the most important person. This is about and it is
00:11 - 00:12
The patient.
00:12 - 00:15
So we have a 50 year old male admitted to the hospital due
00:15 - 00:20
to SBP um spontaneous bacterial peritonitis and
00:20 - 00:21
also had a renal transplant performed.
00:24 - 00:26
So we're gonna get into a little bit of path of physiology.
00:26 - 00:29
Um Sbp is a bacterial infection of acidic fluid
00:29 - 00:33
caused by gram negative bacteria of intestinal origin.
00:33 - 00:36
So basically SPP is an acute bacterial infection of
00:36 - 00:39
acidic fluid that's collected in the peritoneal cavity.
00:40 - 00:44
No source of the infecting agent is easily identifiable.
00:44 - 00:45
Which is what makes it spontaneous.
00:46 - 00:50
It wasn't quite proven in my patient what may have caused this.
00:50 - 00:54
Um Although I know this is not in our scope of practice as nurses.
00:54 - 00:58
I didn't ask my precept of what she may have thought maybe in the cause
00:58 - 01:00
and she believed that it may have been
01:00 - 01:04
a result from him receiving peritoneal dialysis.
01:05 - 01:07
So this next slide is just a cute little
01:07 - 01:10
mean um basically showing you what SmTP looks like.
01:13 - 01:13
Spp.
01:14 - 01:16
Are you sure it isn't just too many donuts.
01:16 - 01:21
So no we most definitely no it's not too many donuts but basically Spp.
01:21 - 01:25
Um when that fluid is built up the stomach is very much floated.
01:25 - 01:28
Um That is exactly how my patient looks
01:28 - 01:31
um on the bed. It looked very uncomfortable
01:32 - 01:34
but that is a representative
01:35 - 01:36
what it looks like.
01:38 - 01:41
So now we're going to get into a little bit of the patient's history.
01:42 - 01:47
So again he um was on peritoneal dialysis hypertension.
01:47 - 01:51
The lady ground function of kidney, epic gastric pain,
01:52 - 01:58
E. S. R. D. End stage renal disease on dialysis, S. B. O. Small bowel instruction
02:00 - 02:02
Trumbo site open Oea.
02:02 - 02:05
And if you do not know what that means, that is basically a low blood
02:05 - 02:06
platelet count,
02:08 - 02:13
hyperglycemia, metabolic acidosis, that section of the abdominal aorta,
02:13 - 02:15
hypoglycemia and anemia.
02:19 - 02:22
So this here is basically just a chart
02:22 - 02:24
showing you what the different cars looked like.
02:24 - 02:27
Um The hockey sticks car which is his rental transplant.
02:27 - 02:31
That is what that looked like. His peritoneal dialysis scars.
02:32 - 02:34
Um when they had those inserted.
02:36 - 02:40
Not quite sure if he had a tunneled cuffed cap there.
02:40 - 02:42
So I'm not going to hold to that.
02:43 - 02:44
Um
02:44 - 02:47
But this next picture is what he,
02:47 - 02:53
what his dissection of the arterial eight order looked like.
02:53 - 02:57
So on top of having a scar on the side. He had this car going down the middle as well
02:59 - 03:00
on top of
03:01 - 03:01
the
03:02 - 03:03
bloated stomach.
03:08 - 03:09
Yeah.
03:10 - 03:10
Okay
03:11 - 03:14
so I'm gonna get into some lab. So um
03:15 - 03:22
His labs for his CBC, his red blood cell count was 2.76 which the normal range is 4.5
03:23 - 03:25
To 5.5. So that is slow.
03:25 - 03:29
Um we do know that red blood cell delivers oxygen throughout the body.
03:29 - 03:33
His hemoglobin was a 5.8 normal 13
03:34 - 03:36
A 13-17 which again very low.
03:36 - 03:41
And we know that hemoglobin is the protein in your body that hosts the oxygen
03:41 - 03:48
And his hematocrit 17.8 and the normal is a 40.50 again. Hello.
03:49 - 03:53
Um So a low score of hematocrit is a sign of
03:53 - 03:58
not having enough iron which is what helps the body make our
03:58 - 03:59
Bc.
04:00 - 04:06
Um So basically how we ended up trying to get these labs back to a normal level.
04:06 - 04:10
We had to give him a blood transfusion.
04:11 - 04:11
And as we know,
04:11 - 04:14
blood is a main important source of us
04:14 - 04:16
being able to survive through the circulation.
04:17 - 04:21
Um the next important lab would have to be the tackle alignments levels.
04:21 - 04:27
Um tackle Lima's. His level was a 4.5. The normal is a 5-20.
04:28 - 04:28
So it's an
04:29 - 04:33
immunosuppressant drugs which is important for kidney transplant
04:34 - 04:38
plant recipients to take to prevent rejection.
04:39 - 04:42
So when the numbers are out of range they are
04:42 - 04:44
more than likely to risk um
04:44 - 04:46
a risk factor for acute rejection.
04:48 - 04:49
So
04:49 - 04:54
to get these levels back you just have to do some heavy therapeutic drug monitoring.
04:54 - 04:57
Um Different meds can affect these levels so you
04:57 - 04:59
have to adjust them according to the patient.
05:03 - 05:04
Thank you
05:05 - 05:06
Some medications.
05:07 - 05:07
Mhm.
05:07 - 05:11
His medications consisted of a sentimental 15 Tylenol
05:11 - 05:14
which was to treat um matter moderate pain,
05:15 - 05:20
carvedilol. Oh um my name is correct. Which is a beta blocker.
05:20 - 05:23
We used to treat heart failure and high blood pressure.
05:24 - 05:26
I'm sorry if I slaughtered this but
05:27 - 05:31
a poet and alpha E. P. B. X. Um Also known as redecorate
05:32 - 05:35
um That is a it throw polices stimulating agent.
05:35 - 05:40
Um This injection was for treatment of anemia due to chronic kidney disease
05:41 - 05:43
that he had um
05:44 - 05:45
own or of dialysis
05:48 - 05:53
heparin. Um anticoagulants which was used to prevent and treat the blood clots
05:53 - 05:55
in the legs and the lungs.
05:56 - 05:57
Um
05:57 - 06:03
Which is good because he was already um not moving as much as he needed to
06:05 - 06:07
and on top of the fluid build up
06:09 - 06:14
um might go ventilate. Um Also known as sales Steps
06:17 - 06:19
works on the immune system to prevent rejection
06:19 - 06:22
after a kidney heart or a liver transplant.
06:22 - 06:24
He was on pro
06:26 - 06:29
cardia X. L. Also um knife DP.
06:30 - 06:33
Um This is an immunosuppressive um It works
06:33 - 06:35
on the immune system to prevent rejection.
06:37 - 06:37
I'm sorry.
06:37 - 06:41
It treats symptoms of angina which is chest pains,
06:41 - 06:43
high blood pressure and pulmonary
06:43 - 06:44
hyper
06:45 - 06:48
attention. And it's actually an antihypertensive drugs.
06:50 - 06:55
predniSONE delta zone which is a cortical steroid for his case.
06:55 - 06:57
It was used to treat blood disorders and to
06:57 - 07:01
um suppress the immune system in order to prevent organ rejection.
07:05 - 07:06
Self a method
07:06 - 07:10
stocks song try method Brain. Um I sat on his bathroom.
07:11 - 07:16
Um This combination is an antibiotic um prevents numerous psychosis.
07:16 - 07:20
Carney pneumonia which is a very severe kind of pneumonia and is
07:20 - 07:24
more common in patients whose immune systems are not working normally.
07:24 - 07:25
Such as those
07:25 - 07:27
patients who have transplants.
07:28 - 07:33
Um You see this word again which is Tech Olympus which is also known as a program.
07:34 - 07:38
It is an immune oppressive is used together with other
07:38 - 07:42
medications to prevent body from rejecting the kidney transplant.
07:43 - 07:44
The last thing was the valves in
07:44 - 07:47
Sis Lover um also known as calcite.
07:49 - 07:50
The first choice.
07:50 - 07:53
This is the first choice antiviral medication used for
07:53 - 07:57
treating infections caused by viruses and preventing C.
07:57 - 07:57
M. V.
07:57 - 07:59
Um Sidle Medal Medal,
07:59 - 08:04
a virus which is an infection in the iron patients who received organ transplants.
08:08 - 08:13
So potential problems um Again failure a rejection of the kidney bleeding.
08:13 - 08:16
Risk for infection um In the septic shot
08:17 - 08:17
shock
08:19 - 08:20
um
08:21 - 08:24
the risk for infection is very high because of his head.
08:24 - 08:27
Open wounds um that needed to be packed and cleaned
08:27 - 08:31
to prevent any other infections on top of the kidney
08:32 - 08:33
um
08:34 - 08:36
whom incision as well.
08:36 - 08:41
Um So septic shock is basically a life threatening condition that happens
08:41 - 08:45
when the blood pressure drops to a dangerously low level after an infection
08:46 - 08:49
um kidney transplant recipients are likely to
08:50 - 08:52
infections due to immunosuppressive therapy.
08:52 - 08:57
So bacteria remains affected to septic shock in these patients
08:58 - 09:02
and that is my presentation and thank you for listening
