00:01 - 00:05
Hello, my name is Alexis and I'm a graduate student at Suny Delhi.
00:05 - 00:07
I'm a critical care registered nurse and I'm here
00:07 - 00:09
today to present my improvement project to you.
00:10 - 00:12
It is so important that we adjust and improve throughout
00:12 - 00:16
the organization to better improve patient care and nurse satisfaction.
00:17 - 00:19
I'm so very thankful for you taking the time out
00:19 - 00:21
of your busy day to be here for my presentation.
00:21 - 00:22
Thank you.
00:27 - 00:31
The COVID-19 pandemic has had an impact on nursing burnout syndrome
00:31 - 00:33
and the number of nurses leaving the bedside has increased.
00:33 - 00:37
This resulted in an ongoing nursing shortage within hospitals nationwide.
00:38 - 00:42
Too much stress exhaustion and low job satisfaction can lead to burnout syndrome.
00:42 - 00:45
As seen at mid Hudson Regional Hospital. Over the past few years,
00:46 - 00:49
one of the medical surgical units that was discussed during an interview with
00:49 - 00:53
the nurse executive Kristen Lawton had lost over 90% of their staff.
00:54 - 00:54
Therefore,
00:54 - 00:57
most of the nursing staff on this unit are brand new and are
00:57 - 01:01
unfortunately having to care for upwards of 10 patients on some shifts.
01:02 - 01:04
The way the assignments are created are not zoned.
01:04 - 01:08
This means that the nurses may have patients scattered all throughout the unit
01:08 - 01:11
not only causing a stressful shift but also placing patients at risk.
01:12 - 01:13
By zoning. The patients,
01:13 - 01:15
nurses would be able to provide better care
01:15 - 01:18
to their patients while improving patient safety,
01:18 - 01:19
decrease adverse events.
01:19 - 01:22
And perhaps improving the nurse's job satisfaction.
01:23 - 01:26
Zoning patients would mean that a nurse's assigned patient load
01:26 - 01:28
are all within the same area on the unit.
01:28 - 01:30
If patients are not receiving adequate care,
01:30 - 01:34
this can affect the organization's reputation when shared with the public,
01:34 - 01:37
resulting in financial losses due to the community
01:37 - 01:40
seeking health care interventions at other nearby facilities.
01:47 - 01:49
Burnout is work related and caused from the work environment.
01:49 - 01:53
This is leading to decreased job satisfaction of nurses,
01:53 - 01:56
poor patient care and unfortunately has negatively impacted
01:56 - 01:59
nurse retention rates on the medical surgical units.
01:59 - 02:02
The turnover rates are extremely high as mentioned earlier,
02:02 - 02:06
with over 90% of the medical surgical registered nurses leaving the bedside.
02:07 - 02:10
This has resulted in new staffing being hired and
02:10 - 02:12
the need for change to occur within the facility.
02:12 - 02:15
If things are not changed and improved within the work environment,
02:15 - 02:18
more nurses will continue to suffer from burnout and
02:18 - 02:20
ultimately leave their position as a med search nurse
02:21 - 02:23
by zoning the patient load.
02:23 - 02:24
Among these units,
02:24 - 02:27
the nurses can have an improved workload and improve
02:27 - 02:29
the care that is being provided to their patients.
02:35 - 02:36
The aim of this study is to reduce medical
02:36 - 02:40
surgical nursing burnout rate by 25% within six months.
02:41 - 02:44
The burnout rates will be measured within all the medical surgical units.
02:44 - 02:45
At Mid Hudson Regional Hospital.
02:46 - 02:49
The three objectives training on burnout for employees,
02:49 - 02:52
mentorship program for registered nurses and zoning.
02:52 - 02:55
The units will be explained in detail on the next few slides
02:59 - 03:00
since nurses are among one of the
03:00 - 03:03
highest risk professions to experience burnout due
03:03 - 03:04
to providing care to sick and dying
03:04 - 03:06
patients while working in stressful environments.
03:07 - 03:10
It is very important to educate the staff on their own risks.
03:10 - 03:14
The working conditions can affect nurses mental and their emotional
03:14 - 03:16
health which may increase the risk of experiencing burnout.
03:17 - 03:20
The approaches to address stress and prevent further burnout.
03:20 - 03:22
According to evidence based practice
03:22 - 03:26
includes educating and making providers aware of their own potential,
03:26 - 03:29
promoting positive mental health and creating environments
03:29 - 03:31
with controlled stress.
03:32 - 03:34
Laurie head of education will assist in
03:34 - 03:36
creating the curriculum to provide the education
03:37 - 03:38
for the staff
03:43 - 03:46
using yoga to prevent and manage stress and burnout among health
03:46 - 03:50
care workers has been proven by systemic reviews of literature.
03:50 - 03:52
Yoga has been proven to be effective
03:52 - 03:55
in managing stress and improving physical emotional
03:55 - 03:57
and mental health among nurses which therefore
03:57 - 03:59
can decrease the occurrence rate of burnout
04:00 - 04:02
by increasing yoga and meditation.
04:02 - 04:06
Healthcare workers can be aided in achieving a stable psycho physical well being
04:07 - 04:10
which enhances their value within the workplace environment
04:15 - 04:17
by assigning patients within a zone.
04:17 - 04:20
Nurses can work in a safer less stressful
04:20 - 04:22
environment while working together as a team.
04:23 - 04:25
The wasted time spent by nurses walking across
04:25 - 04:28
the unit contributes to increased fatigue workload and
04:28 - 04:30
stress which therefore can lead to higher levels
04:31 - 04:34
of burnout and negatively affect job satisfaction.
04:34 - 04:36
These unnecessary movements impair the overall
04:36 - 04:38
performance of nurses on the units.
04:39 - 04:42
It was determined that environmental influences the nurse's
04:42 - 04:45
satisfaction and therefore impacts the patient care provided.
04:49 - 04:52
The nurse educators would not cost any additional money for this
04:52 - 04:55
study since their role is part of their usual duties.
04:55 - 04:57
This is true for the librarian,
04:57 - 05:00
nurse manager and the registered nurses involved as well.
05:00 - 05:04
Therefore, the only additional cost would be the yoga or meditation instructor
05:04 - 05:08
which would cost approximately $3840 for six months
05:10 - 05:11
overall.
05:11 - 05:13
This project is relatively affordable.
05:13 - 05:17
Most of the resources and individuals are available within the hospital setting.
05:18 - 05:23
The cost of the project just under $4000 is worth it because retaining nurses and
05:23 - 05:26
providing nurses with a less stressful work environment
05:26 - 05:29
is necessary to prevent nursing burnout rates.
05:29 - 05:31
It is important to implement change within the
05:31 - 05:34
workplace to have change among the turnover of staff
05:35 - 05:37
by improving the workplace environment
05:37 - 05:40
and decreasing stress, perhaps burnout will be decreased.
05:40 - 05:44
And the goal of reducing nurse burnout rates will be achieved.
05:49 - 05:52
Barriers must always be taken into consideration
05:52 - 05:54
when creating a timeline for implementation.
05:54 - 05:58
Although we have dates set for each objective and associated actions,
05:58 - 06:01
it is known that they may have to be altered
06:01 - 06:04
a barrier we may encounter would be a cancellation a
06:04 - 06:06
cancellation of the meeting with stakeholders which
06:06 - 06:09
may delay the start of our project.
06:09 - 06:13
Also, if the educational trainings and curriculum are not completed on time,
06:14 - 06:16
this may also delay the start of the project.
06:22 - 06:25
The burnout rates among the medical surgical nurses will be assessed
06:25 - 06:28
throughout this project to assess the impact of zoning assignments.
06:29 - 06:31
Perhaps these rates will be assessed monthly to keep
06:31 - 06:34
an ongoing assessment on the changes within the organization.
06:35 - 06:37
Both the short term and long term effectiveness
06:37 - 06:40
of creating zones within the medical surgical units will
06:40 - 06:43
have to be assessed throughout the project and at
06:43 - 06:45
the completion of the project to assess the effectiveness
06:46 - 06:49
since the change will not only have the impacts on the nursing staff
06:49 - 06:53
but also the patients being cared for on the medical surgical units.
06:53 - 06:57
It would be beneficial to also conduct surveys among this population.
07:00 - 07:03
This project supports the mission vision and
07:03 - 07:05
value statements of Mid Hudson Regional Hospital
07:06 - 07:08
to provide the highest quality care to residents,
07:08 - 07:12
the nursing staff must be available and if nurses continue to leave the bedside,
07:12 - 07:14
this will not be possible.
07:14 - 07:16
Therefore, by improving the workplace environment,
07:16 - 07:19
nurses will continue to work at the bedside and
07:19 - 07:21
provide patients with the highest level of care.
07:22 - 07:24
In order to be the provider of choice for health care,
07:24 - 07:27
patients must be satisfied with the care provided.
07:27 - 07:30
And by creating zones on the medical surgical units,
07:30 - 07:34
patients will receive more adequate and timely care. While hospitalized.
07:34 - 07:39
Lastly, the value statements, emphasize innovating and improving the network,
07:39 - 07:42
which is the main focus of this project presented here today,
07:47 - 07:49
multiple methods and platforms of communication
07:49 - 07:51
will be used throughout this project.
07:51 - 07:54
This project will be presented on the internet,
07:54 - 07:55
the internet
07:55 - 07:57
that is internal
07:58 - 08:00
for the staff here at mid Hudson regional Hospital.
08:00 - 08:04
Emails will also be sent out to all the nursing employees within the facility.
08:05 - 08:06
And in each patient care room,
08:06 - 08:10
there will be a small postcard explaining the changes that are occurring
08:10 - 08:13
and encouraging patients and their family members
08:13 - 08:16
to provide their support feedback and suggestions
08:16 - 08:18
with contact information for the nurse manager.
08:19 - 08:21
There will be a monthly meeting throughout the project to
08:21 - 08:25
discuss project success and where improvements may be needed.
08:25 - 08:28
There will be multiple surveys conducted and even
08:28 - 08:30
private interviews with nurses on the units.
08:35 - 08:37
The work environments are stressful and nurses are
08:37 - 08:39
leaving the bedside at an increased rate.
08:39 - 08:42
This is resulting in organizations to spend more
08:42 - 08:44
money on hiring and training the new staff.
08:46 - 08:48
This project is necessary
08:48 - 08:52
to improve the work environment for nurses to lead
08:52 - 08:54
to better retention rates and lower the burnout rates.
08:54 - 08:56
Among these nurses,
08:56 - 08:59
we need the nursing staff to provide patients with care.
08:59 - 09:01
Please join us at the next meeting
09:02 - 09:06
in two weeks to discuss the initiation of the zoning on the medical surgical units.
09:06 - 09:07
Once again,
09:08 - 09:10
I appreciate your time here today and I look forward
09:10 - 09:13
to working with you all over the next several months.
09:13 - 09:15
To better the organization.
09:16 - 09:19
If there are any questions, I would be happy to answer them. Now.
09:19 - 09:20
Thank you.
09:27 - 09:31
Here is a brief list of the references that were used during this presentation.
09:32 - 09:33
Thank you again.
