  2. My name is Tatiana Gardner and I serve is the
  3. community liaison for the Fulton DeKalb Hospital Authority.
  4. I have been tasked with fostering a working relationship with
  5. our community and the patients we serve in order to
  6. advance health equity.
  7. In order to do so, I have established the Community
  8. Coalition Board.
  9. The primary goal of this board will be to establish
  10. a healthy relationship with Community Organizer's in order to ensure
  11. that every individual in this community has access to become
  12. the healthiest versions of themselves.
  13. The three pillars of this board will be to engage,
  14. collaborate and provide engage.
  15. As part of this board, we will be sure to
  16. engage and involve community members and organizations in our strategic
  17. visioning in planning.
  18. Since targeting social determinants of health equity will be a
  19. goal of ours, it is imperative that the community is
  20. involved in order to address all the issues.
  21. Even if we're not made away with them collaborate, we
  22. will collaborate with worker centers and community organizations to document
  23. health, living and working conditions among the low wage and
  24. the vulnerable populations that we serve provide we will be
  25. sure to provide resource is to organizing groups working on
  26. policy campaigns to make the connection between health and the
  27. issues that they care about.

  29. board because advancing health equity is a primary goal of
