  1. This video is intended to give you a general overview
  2. of the state survey process and the copy program here
  3. at mountain view manner.
  4. I will discuss the purpose of surveys and how to
  5. prepare for your part of the process.
  6. I will give you some important tips to help with
  7. surveyor questions and requests.
  8. Then I will discuss the quality assurance performance improvement program,
  9. including the what and how of Coffee CMS.
  10. State surveys govern the delivery of care and services within
  11. the organization.
  12. These guidelines are in place to ensure that all residents
  13. receive the highest quality of care.
  14. There are two types of inspections or surveys.
  15. The first is an annual survey that happens every year
  16. and takes about 3-4 days unless surveyors have concerns in
  17. which it may be longer surveys can happen any time
  18. during the year, at any time of the day in
  19. any day of the week.
  20. They are always unannounced.
  21. The second type of survey is a complaint survey and
  22. that's a survey that arises following a specific complaint against
  23. the facility and it looks specifically at areas that are
  24. related to that complaint.
  25. It will not be made known who filed the complaint
  26. and you will also not know when it will occur.
  27. These types of surveys typically last 1-2 days and they
  28. are complete when enough evidence is gathered to determine if
  29. the complaint is valid regardless of the survey type.
  30. Upon completing the survey, the surveyors were will decide if
  31. any areas do not meet minimum health and safety standards
  32. in unacceptable conditions.
  33. The surveyors will issue a violation.
  34. Also known as a citation tag deficiency or a finding
  35. For each violation.
  36. Your organization received.
  37. The administrator along with nursing staff must develop and then
  38. submit a plan of correction within 10 days after review
  39. of the pOC or plan of correction, a post survey
  40. revisit maybe completed to verify the organization corrected the violations.
  41. It also may not be revisited if the plan of
  42. correction meets the surveyors requirements and then depending on the
  43. survey history and the type of violations that were cited.
  44. Yeah. Some common tags or deficiencies um that relate to
  45. the survey visits include residents rights, infection control, mealtimes and
  46. dining the environment and plan of care.
  47. So we're just going to talk a little bit specifically
  48. about some of these tags and how we can avoid
  49. them. The first one.
  50. Residents rights referring to the right to dignity, privacy and
  51. respect ways to avoid this violation.
  52. In regards to breaching residents dignity privacy um include always
  53. knocking and giving residents a chance to respond prior to
  54. entering their private room when addressing residents, always be respectful
  55. called them by the name that they wish to be
  56. called. And again, always avoid pet names, provide privacy and
  57. avoid exposing body parts unnecessarily when you're doing your cares
  58. always dress residents in their own clothes and no nighttime
  59. clothing in the day unless this is their preference, in
  60. which case it should be care planned.
  61. Urinary catheter bag should be covered at all times.
  62. We never changed the tv or radio to our preference
  63. but rather leave it at the preference of the residents.
  64. The second common tag that is often given, relates to
  65. infection control this year, a very common violation or tag
  66. is given in relation to hand washing.
  67. So that's why we've been so often going over our
  68. hand washing policies and practices and again encouraging to do
  69. you to rub in and rub out and knowing those
  70. five points of contact when you need to be washing
  71. your hands.
  72. Um Please please don't miss an opportunity to wash your
  73. hands. Uh huh.
  74. Especially during the survey process if you're not sure, just
  75. wash your hands.
  76. A second infection prevention tag that's commonly coming as in
  77. regards to PPE.
  78. Or personal protective equipment.
  79. So make sure you're always dressed in your correct PPE
  80. for the care situations that you're in.
  81. And importantly, please dispose of your PPE in the appropriate
  82. receptacles. Never walk in halls while wearing PPE, never place
  83. soiled linens on the floor or on clean surfaces.
  84. They should be placed in bags for transport outside of
  85. the residents rooms.
  86. In regards to mealtimes and dining citations.
  87. These can be avoided by making sure that your hands
  88. are washed as well as a residence part eating, making
  89. sure that the food corresponds to the specific diet of
  90. the resident.
  91. Always ensuring that residents receive appropriate level of support and
  92. have their assistive devices that they have been care plan
  93. for. And when you are assisting with eating, sit next
  94. to the resident, don't ever stand over them or reach
  95. across tables to them.
  96. If a resident is refusing what they've been offered to
  97. eat, you can please offer food substitutes and then also
  98. determined do they need assistance?
  99. Um, so don't just take the first refusal look for
  100. ways to encourage leading talk to residents and meals even
  101. if they cannot respond, please do not carry on conversations
  102. with other employees.
  103. This will result in a citation and finally, um, toilet
  104. residents and provide personal hygiene.
  105. Again, the hand washing before you take them to the
  106. dining room for mealtimes and again, personal hygiene Afterward.
  107. Finally, you know, use respectful terms.
  108. Do not use terms like feeders or bibs.
  109. Don't ever serve the meal on the food tray and
  110. temperature is important.
  111. And in the past, received citations for this.
  112. So reheat as needed if the resident is eating too
  113. slowly. Tax related to the environment, to deficiencies in the
  114. safety cleanliness and home like nature of the environment.
  115. Be certain to pick up after yourself when giving care,
  116. check the residents room to ensure it is neat before
  117. leaving. Make sure items are within reach, like the call
  118. light and the water picture in their remote control.
  119. Check for and always remove safety hazards.
  120. Keep the noise level within the environment to a minimum.
  121. Don't ever yell in the hallways to a coworker or
  122. resident and finally a common deficiency.
  123. Nursing homes received from surveyors related to plan of care,
  124. surveyors will typically ask about planet cares.
  125. They may ask you, how does the nurse communicate the
  126. residents, changes such as illness or weight loss and how
  127. they are addressed and documented.
  128. So you should remember that at mountain view manner.
  129. We communicate care plan updates during reports, Also during huddles
  130. and in our sienna communication binder that you guys use.
  131. And we will be implementing the use of the facility
  132. bulletin board in the Pcc dashboard.
  133. Um so when you log on to pcc you'll see
  134. on your dashboard there's that home tab before you go
  135. to pcc.
  136. When there are updates in this, this home tab turns
  137. red when there are new updates.
  138. And until you look at it then after you look
  139. at it will turn back to white.
  140. So we want to begin using this communication tool at
  141. the start of each shift.
  142. And we want to all know that this is where
  143. we communicate the care plan changes.
  144. So how should you conduct yourself during the survey?
  145. First of all to stay calm, you guys do a
  146. great job and be confident in the job that you
  147. do maintain your positive carrying attitude.
  148. Smile, Be friendly to the surveyors.
  149. Be cooperative when you are approached and asked questions of
  150. them. Do not ever guess if you don't know the
  151. answer or can't remember.
  152. That's fine.
  153. Just always know that you know can express to them
  154. that you know where to find the answer.
  155. So it's okay to say, I'm not sure.
  156. But let me get the person who can best answer
  157. your question or since you have me on the spot,
  158. I just can't remember but I know where to find
  159. the answer.
  160. Let me find the answer for you.
  161. Um Mind you surveyors are going to expect you to
  162. be able to answer certain questions such as those surrounding
  163. fire safety and abuse.
  164. So these questions need to be ingrained in your mind.
  165. You need to be able to to answer these questions
  166. When you ask questions stick to you know the question
  167. at hand less is more.
  168. Don't just start rambling on about all kinds of other
  169. things. And then finally if you are asked by a
  170. survey to perform a demonstration you can you will but
  171. please notify your D.
  172. O. N.
  173. Prior to that.
  174. So some common questions include um that you may be
  175. asked by surveyors.
  176. To who do you report suspected cases of your views
  177. and you need to know that you immediately report those
  178. to your supervisor which maybe your charge nurse or deal
  179. in or your administrator.
  180. You need to know the types of abuse and the
  181. signs and symptoms of abuse.
  182. We've talked a lot about this.
  183. You do an annual course for this.
  184. If you have questions, please see me regarding this, you
  185. need to know what happens when someone accuses an employee
  186. of abuse and that is that the employees suspended pending
  187. an investigation.
  188. And um you also need to have committed to memory.
  189. What is the first thing that you should do in
  190. case of a fire is protect your residents.
  191. And then the race acronym remove alarm contained, evacuate.
  192. And also you need to know how to use the
  193. fire extinguisher with the past paul ames squeeze sweep acronym.
  194. Okay, so now we're going to quickly just talk about
  195. the coffee program.
  196. So while you already may have an understanding of what
  197. this program is about, I'd like to just take a
  198. couple minutes to discuss the how and why of coffee.
  199. So again, copies Q.
  200. A. P.
  201. I. Stands for Quality Assessment Performance Improvement.
  202. The federal government requires that every location that participates in
  203. Medicare and Medicaid programs implement a quality program.
  204. The goal of the Q.
  205. A portion of copy, which stands for quality assurance or
  206. quality assessment, is to prevent poor quality care and services
  207. from reaching the people that we serve.
  208. The P.
  209. I. In copy references.
  210. Performance improvement, which aims to solve the problems that are
  211. identified by the quality assessment.
  212. The coffee program is directed by a team of coffee
  213. committee members that include our administrator dan, the director of
  214. nursing, Janet, Medical director, DR Diana Carr V.
  215. And Becky Gray, our new social worker who is our
  216. designated coffee Coordinator.
  217. These team members meet monthly where they reviewed data and
  218. audits and developed tracking tools to determine areas of improvement
  219. that are needed within the center.
  220. Through their review, they determine significant areas of concern and
  221. then develop performance improvement plans, also known as pIPs, to
  222. solve these problems and improve care.
  223. Any Mountain View Manor staff who has input or a
  224. special interest in finding solutions to a particular problem identified
  225. by the committee can and should be part of the
  226. performance improvement team.
  227. The principles of the Coffee Program aimed to encourage employees
  228. to identify opportunities for improvement and share ideas for change.
  229. The focus is on improving systems and processes rather than
  230. blaming individuals when things go wrong.
  231. The copy plans guide our work and help us to
  232. clearly define goals, review those goals and update them as
  233. needed. So please remember that copy is everyone's responsibility.
  234. Everyone is expected to know what coffee projects are going
  235. on and know that each of us has a voice
  236. in improving our center and providing the best care possible
  237. to the residents that we care for.
