  1. hello fellow brand and colleagues and Dr William Mira's My
  2. Name is Alice Acea.
  3. I will be discussing the heart failure Health campaign.
  4. Let's get started, What's on the agenda?
  5. So we'll be discussing the overview the population at risk,
  6. health risk, communication and compliance, theoretical framework, multimedia resources and
  7. my references.
  8. Chronic heart failure is a serious clinical public health problems
  9. associated with significant mortality and morbidity.
  10. I have been working with matters of the heart since
  11. the beginning of my career.
  12. I worked in a heart failure progressive care unit as
  13. an RN, and now I am one of the interventional
  14. cardiology MPs at Providence Specific Heart Institute.
  15. So why is this important?
  16. About 6.5 million American adults have heart failure, and the
  17. lifetime risk of heart failure is about one in five.
  18. At the age of 40 years old, 20 to 46%
  19. of heart failure patients display poor treatment, adherence with medication
  20. and self care, which leads to preventable hospitalizations, increased health
  21. care costs, poor quality of life and increased mortality.
  22. Yeah, heart disease in Los Angeles County and sub populations
  23. in 2017, the leading cause of death amongst residents in
  24. L a county was related to coronary heart disease, estimating
  25. about 63,429 deaths.
  26. Coronary heart disease mortality was about two times higher for
  27. African Americans and 1.5 times higher for Caucasians than Latinos
  28. and Asians of adults about 40 years old and above.
  29. One in five Americans will develop heart failure in their
  30. lifetime health risk.
  31. People diagnosed with heart failure have compromised physical health, since
  32. the heart has the inability to meet the metabolic needs
  33. of the body.
  34. The incidence of heart failure increases with age, and its
  35. prevalence is projected to increase by 46% by 2030.
  36. Medical costs were estimated to be 30.7 billion in 2012
  37. and projected to be 69.7 billion by 2030 in 2012.
  38. CMS initiated the Hospital readmissions reduction program by disciplining hospitals
  39. with a surplus of 30 day readmission rates, which have
  40. definitely helped decrease the rates.
  41. Mm hmm.
  42. Bridging the communication, the online portal serves as a platform
  43. not only for the communication between the clinician and patient,
  44. but also as a resource for patients to be properly
  45. educated with her diagnosis with substantial patient education.
  46. This action serves as an effort to reduce the risk
  47. of readmission to vulnerable populations diagnosed with heart failure.
  48. By empowering patients to understand their self care needs, the
  49. three R's are review, reassess and re educate.
  50. Each patient requires an individualized plan for successful transition from
  51. the hospital to home and preventing readmission.
  52. Clinicians must review the patient's current plan of care and
  53. adherence to it.
  54. In addition, clinicians must reassess the current plan with a
  55. patient and family.
  56. To ensure that the plan continues to meet the patient's
  57. needs, clinicians must continually re educate patients about their plan
  58. of care, their plan for self management and strategies to
  59. prevent hospital re admissions for heart failure.
  60. I chose Dorothy A.
  61. Orem Self care theory in the setting of heart failure.
  62. Dorothea Orem Self care deficit theory focuses on each individual's
  63. ability to perform self care, defined as the practice of
  64. activities that individuals initiate and perform on their own in
  65. maintaining life, health and well being.
  66. So heart failure Self care includes weight monitoring, taking multiple
  67. medications, symptom management, increased physical activity, dietary restrictions, specifically sodium
  68. intake. Oram's theory serves as a foundation to classify an
  69. individual strengths and aptitudes efficiently direct self care assistance with
  70. self care deficit and nursing systems application to meet individual
  71. needs and requisites.
  72. The four meta paradigms include health, patient and individual environment
  73. and nursing multimedia approach.
  74. The best multimedia approaches I've seen include the cardio visual
  75. app videos, social media, back sheets, brochures and the Woman's
  76. Health Symposium and the Heart Failure Symposium.
  77. The cardio visual platform provides clinician validated video content through
  78. the mobile and integrated solutions that help healthcare providers effectively
  79. deliver interactive information to patients and other clinicians and empower
  80. them to make better decisions and improve health outcomes.
  81. The Break up with Salt Campaign, shown here is by
  82. the American Heart Association.
  83. The American Heart Association launched a consumer education campaign encouraging
  84. Americans to break up with excess salt.
  85. It seeks to build an army of passionate and willing
  86. supporters to pressure food manufacturers and industries to reduce sodium
  87. in packaged foods.
  88. Here we have a self check plan, so the self
  89. check plan fact sheet shows heart failure, symptoms and the
  90. three tiers, excellent self care, cautionary symptoms and management and
  91. medical alert warnings.
  92. And then here we have a little social media posts
  93. saying that 26 million people with worldwide are living with
  94. heart failure, and then the heart failure symposium is still
  95. available. Um, via Zoom, I believe, or through, um, through
  96. Microsoft teams.
  97. And it's on March 12th and 13th.
  98. If anyone is interested.
  99. Yeah, yeah, okay, this is the Social Media Post I
  100. created giving simple heart failure tips.
  101. It's easy to understand and gives clinicians and patients basic
  102. educational tips to help with heart failure.
  103. One being less salt, more exercise as tolerated, of course,
  104. Take meds as prescribed smoking cessation, limiting alcohol and weighing
  105. yourself everyday Mhm.
  106. Yeah, here are some resources and links.
  107. One is the skip the salt campaign, and then the
  108. second is with the American Heart Association.
  109. Uh, W w dot heart dot org More about healthy
  110. eating Here is the link for the cardio visual app.
  111. If you decide to, um, better engage your patients, uh,
  112. during, uh, the educational part of the visit, uh, you
  113. could just show them what the diagnosis is.
  114. Some of the medication and it gives them different videos
  115. to look at.
  116. And then the last link is the heart Failure Symposium
  117. that I previously spoke about.
  118. It's on March 12th and 13th for clinicians so that
  119. you can be more educated on heart failure and it
  120. gives you more teaching tools for your patients.
  121. And here are my references.
  122. Thank you so much for listening.
  123. Have a great day.
  124. Evening, wherever you are, take care.
