00:00 - 00:03
Hi everybody at Rockford Hospice Loren Bouchard here to talk to you about
00:03 - 00:07
your patient experience scores for the month of april let's have a look.
00:09 - 00:14
Okay? So you can see here you guys are at 83.33, you dropped down a little bit,
00:14 - 00:16
you had 12 surveys returned.
00:16 - 00:20
Um but we're gonna take a look and see you're not meeting your target this month,
00:20 - 00:22
but we'll definitely see like what's holding that
00:22 - 00:24
up because you did hit your target last month
00:25 - 00:28
and the month before. So let's take a look.
00:28 - 00:32
So here's the breakdown of the survey returns. Um the top box.
00:32 - 00:37
So you had nine surveys that gave you a 10 1 survey gave you a nine.
00:37 - 00:43
So those were both top box but everything below that. Not so eight you had 18 and 15.
00:44 - 00:48
So just to think about that, you know, if we could influence those AIDS, you know,
00:48 - 00:51
how do we influence the eights to give us a nine or
00:51 - 00:55
a 10 and sometimes that's what we need to be thinking about.
00:55 - 00:58
It's usually just consistency and um communication,
00:58 - 01:01
all the things we talked about, but you're doing great overall.
01:01 - 01:06
If you look at your overall target here, you're 89.47. So just
01:06 - 01:10
A hair below your target of 89.56.
01:10 - 01:10
It's shown it in green,
01:10 - 01:15
but it's really read until you hit that um until you hit that note.
01:15 - 01:17
So which is literally
01:17 - 01:18
it's what point
01:19 - 01:22
.9.09 I think to get to that.
01:22 - 01:26
So um if we go up next month um you'll be
01:26 - 01:29
at your target for overall for patient experience year to date.
01:29 - 01:32
And you can see um as an entity
01:32 - 01:34
hospice, as an organization,
01:34 - 01:39
as a service line is at their target this month and for four months in a row.
01:39 - 01:41
Which is super exciting.
01:41 - 01:44
The greatest increases trained to safely move,
01:44 - 01:47
trained to help with restlessness and agitation.
01:47 - 01:49
Given confusing information about care.
01:49 - 01:52
Uh get as much help with pain as needed and identify
01:52 - 01:55
pain med side effects have all improved since last month.
01:55 - 01:58
Greatest decreases nursing home information, work together,
01:58 - 02:03
help with trouble with constipation, get help for trouble breathing,
02:03 - 02:08
hospice team help after hours and information on if and when to get pain meds.
02:08 - 02:09
Those decreased
02:09 - 02:11
from last month, this month.
02:12 - 02:16
Your top priority index listening carefully to you number one.
02:16 - 02:20
Keep informed about care number two and emotional support number three,
02:20 - 02:23
which we're going to focus in on this month, emotional support received.
02:23 - 02:28
So just making sure you, your patients and families feel like a priority.
02:28 - 02:28
You know,
02:28 - 02:30
when they're talking about something and we want to
02:30 - 02:32
make sure we validate their feelings and thoughts,
02:32 - 02:34
avoid interrupting them
02:35 - 02:39
and just providing that empathy and compassion towards their questions and
02:39 - 02:43
concerns maintain that positive body language avoid crossing your arms,
02:43 - 02:46
watch your facial expressions and if you can commit
02:46 - 02:49
to sit when possible and give eye contact.
02:49 - 02:54
That is really, really helpful tool. Um and feeling for them. Feeling supported,
02:54 - 02:59
assist them with preparing for their loved one's death by making sure we, you know,
02:59 - 03:02
bring out the gun from my sight booklet and walk through that, you know,
03:02 - 03:06
point out different areas maybe where their loved one is in the process.
03:06 - 03:09
Um, and then you can always offer additional resources.
03:09 - 03:10
We have social workers,
03:10 - 03:13
we have chaplains just being sure to offer
03:13 - 03:16
those services and last but not least we have
03:16 - 03:20
family comments and if you want to pause your screen to take a look at that,
03:20 - 03:25
I see a few little call outs for Angie and Minerva to reverend Darren as well.
03:25 - 03:30
Um so great work there And uh just keep up the great work, everybody.
03:30 - 03:34
This is an awesome group of people and I know um you're just so close
03:34 - 03:39
to hitting 118 to a nine would have taken you there to your target.
03:39 - 03:41
So just kind of keep your eye on the ball.
03:41 - 03:45
Do those communication tactics if you need anything, let me know.
03:45 - 03:47
And I will talk to you next month. Thank you
