  1. Yeah. Hi, my name is Milton Alfonso and I'm here
  2. to talk to you about Ike or the international cardio
  3. sell outcomes registry.
  4. I'm a surgeon at the Queensland Children's Hospital in Brisbane
  5. in Australia.
  6. I moved here in June 2012 from all the children's
  7. hospital in Liverpool in England.
  8. And soon after the movie, I was requested by my
  9. former colleague Tom Karl to consider using calm yourself.
  10. My only condition at the time was that we collect
  11. prospective data and the prospective data collection continues to this
  12. day and has now expanded into something more ambitious namely
  13. I car.
  14. Yeah. Yeah.
  15. So what is cut yourself kill yourself is bovine pericardium
  16. that has been subjected to the adapt and the calcification
  17. process. The adaptive process transforms the bovine pericardium into an
  18. a cellular, strong pliable collagen scaffold all the notices for
  19. calcification including nucleic acids, alpha gal episodes, cells and cell
  20. remnants. And forceful methods are removed.
  21. The pericardium is then strengthened by cross linkage with mon
  22. America Blue Rally hype.
  23. The final step of the process and was detoxification by
  24. removal of the residual value add careers health comes stored
  25. in sailing and is available in different sizes off the
  26. shelf based on our experience with carry yourself over the
  27. past nine years.
  28. We have published three peer reviewed articles on its results.
  29. This was our first paper published in the seminars of
  30. thoracic and cardiovascular surgery in 2017.
  31. This was a purely histological study using conventional and immuno
  32. history chemical staining on six X plants, none of which
  33. were related to call yourself.
  34. And the study demonstrated that there was no calcification in
  35. any of the six implants.
  36. But there were signs of early remodeling with new fibroblasts,
  37. collagen deposition, new and timor formation and new revascularization.
  38. Yeah. This was our first clinical Paper published in the
  39. interactive cardiovascular and thoracic surgery journal in 2018.
  40. It involved 135 patients all from our center.
  41. And every patient in the study had completed 24 months
  42. of flour.
  43. The freedom from re intervention was 96 of two years
  44. and 95 of three years.
  45. There was no difference in performance in the systemic versus
  46. the pulmonary circulation and between units, infants and older Children.
  47. He then went on to publish a multi center study
  48. in the annals of Thoracic surgery in 2019.
  49. The study involved three centers to in Australia, Brisbane, Melbourne
  50. and one in the UK.
  51. The study involved 377% 377 patients.
  52. The freedom from the intervention at three and 5 years,
  53. 96 percent.
  54. Yeah. And once again there was no difference in performance
  55. in the systemic opinion, every position or between the inmates,
  56. infants and all the Children.
  57. Which cardio cell was first developed by a company named
  58. Admiral. Is that is based in Malaga in both in
  59. western Australia.
  60. The factory is still in Malaga.
  61. I have been to the factory.
  62. All the bovine pericardium is sourced locally And 95 of
  63. this bovine pericardium is rejected because they focus on obtaining
  64. patches with uniform thickness.
  65. It's an absolute priority for the company.
  66. And our histological study demonstrated that the cardio, which was
  67. uniformly 400 migrants and thickness carousel nears even thinner.
  68. They select the tennis parts of the pericardium.
  69. It's uniformly 200-300 microns in thickness, making it a good
  70. match for pulmonary artery and wild repairs, especially in the
  71. younger Children.
  72. Uh huh.
  73. I said I alluded to earlier.
  74. We first began using carrier school in October 2012.
  75. 2 centers that began using it at the same time.
  76. One was asked the other was the Royal Children's Hospital
  77. in Melbourne in Australia.
  78. Call yourself is used under the T.
  79. J. The therapeutics Good Administration's special access scheme.
  80. Yeah. Currently it's used in over 25 countries across the
  81. world and it's becoming increasingly recognized as a viable alternative
  82. to other mainstream materials for reconstruction in congenital heart defects.
  83. Mhm. As of today, cardio cell is being used in
  84. prison, pull it Melbourne in Australia.
  85. And the starship turned into hospital which is the only
  86. Children's hospital in New Zealand.
  87. Yeah. So now moving on to I call what distinguishes
  88. the registry is supposed to regional statistics in a registry.
  89. You collect some information on all patients in a research
  90. elevation, collect all the information want some patients in the
  91. registry. Where a systematic collection of a clearly defined set
  92. of health and demographic data that is held in a
  93. central database and its very nature makes it more powerful
  94. than multi center studies.
  95. Yeah. From the perspective of using cardio.
  96. So what is that?
  97. We would like to see from my core the collection
  98. of demographic details diagnosis, risk factor, operative details, morbidity, mortality,
  99. follow up three interventions and any possible astrology from England
  100. on the entire cardio self family.
  101. The data should be collected prospectively and ideally we'd like
  102. longer term follow up.
  103. There are to be entered into icon periodically.
  104. Our vision is to foster systematic multi center collection of
  105. data relating to the implantation of cardio self because people
  106. have large numbers in different economic locations.
  107. This will enable meaningful statistical comparison between groups and then
  108. we can use this knowledge taylor and improve outcomes.
  109. In order for I call to work.
  110. It needs to be based modern technology and huge approved
  111. to a large extent.
  112. Okay, we need a secure online application for building databases.
  113. The application must be highly versatile, must have customizable reporting
  114. functions must walk across various platforms.
  115. Both Mac and hand Roy.
  116. And the data must be exportable into a wide range
  117. of formats so that you can prepare reports or conduct
  118. research studies.
  119. There are several obvious benefits to having a registry of
  120. this nature to describe the use of cardio sail across
  121. several centers.
  122. We can produce annual reports.
  123. Daniel reports will give us basic demographic data uh information
  124. about the utilization of cardio sale in different centers in
  125. different anatomical locations.
  126. Okay. Mhm.
  127. We will be able to determine the early and late
  128. outcomes after surgery using card yourself from a research perspective,
  129. prospectively collected data is very valuable.
  130. Yeah. Okay.
  131. The large numbers of patients and different anatomical locations, different
  132. age groups makes for meaningful statistical comparison.
  133. All this information we'll give true meaning to the word
  134. informed consent.
  135. Yeah, yeah, importantly, the analysis will enable us okay from
  136. a surgical perspective to understand what cardio self doesn't perform
  137. as well as expected and we can tailor our surgical
  138. strategies as necessary.
  139. Okay. From the developers, person developers perspective this is more
  140. like a dream.
  141. Okay, that's good.
  142. And there's another major intangible benefit.
  143. Ah That's a very small subspecialty.
  144. And with I call we get to make friends.
  145. I call already has a government structure in place.
  146. There is a steering committee and the steering committee will
  147. always have either the director or nominated surgeon from each
  148. of the contributing billets.
  149. There's a management team.
  150. Um the data custodian for the database Jessica sooner is
  151. a clinical research manager.
  152. General Johnson is a clinical data manager.
  153. KIM Bets is the statistician, helps generate the reports and
  154. do all the research work that comes out of icon.
  155. It is the responsibilities of the management committee primarily to
  156. communicate with participating centres, provide advice and help with committing
  157. ethics or completing ethics and governance applications, designing content forms
  158. for each institute, validating data as best as possible, generating
  159. an annual report, facilitating research projects and for purely logistic
  160. reasons. We plan to focus on Australia and new Zealand
  161. and the countries in the AsIA pacific region in the
  162. first instance.
  163. But there are no barriers.
  164. Any unit across the world that is using Cardio self
  165. wants to submit.
  166. That is very welcome to join.
  167. Icahn requires ethics approval at the various centers and for
  168. all the units that are currently contributing data.
  169. We not only have ethics are proven, but we have
  170. data sharing agreements across borders.
  171. one big advantage of setting all this up up front
  172. is that it makes engaging in multi center research projects
  173. fairly uncomplicated from a governance perspective.
  174. Okay. Yeah.
  175. From a database standpoint has been developed in Red Cap.
  176. The research electronic data capture.
  177. Red Cap is a secure web application for building and
  178. managing online surveys and data bases.
  179. It was developed by a team at the Vanderbilt University
  180. in Nashville in Tennessee and while it can be used
  181. to virtually collect any type of data in any environment,
  182. it is specifically geared to support online and offline data
  183. capture for research studies.
  184. Okay. The current time, the red crab consortium, which is
  185. a vast support network of collaborators, is composed of 5000
  186. active institutional partners in 141 countries With 1.7 million users.
  187. And this data is accurate.
  188. As of April 2021.
  189. Red gap has several advantages.
  190. From an I cord perspective, it's highly versatile, it's very
  191. user friendly.
  192. I it's free for any users.
  193. All we do is pay a small annual field uh
  194. for housing the server, uh Children's health Queensland or rather
  195. in our hospital.
  196. It works across all platforms and across all devices.
  197. It works on any computer, It works on any laptop,
  198. it works on any mobile phone.
  199. The data is very easily exportable into a wide range
  200. of formats.
  201. You can run customizable creation simply export those queries into
  202. Excel. For example, I wanna really very good example of
  203. an existing international, existing bi national registry that has been
  204. very, very successful uh, is the Australian new Zealand Fontaine
  205. registry. Red Cap my I call is housed on a
  206. secure Children Children's health Queensland.
  207. So we're here in Brisbane as of now, as the
  208. management team.
  209. We are we are moving to face for the Red
  210. Cap database has been designed and tested.
  211. We have all the ethics approvals and network agreements for
  212. data sharing amongst the Australian centers and they a few
  213. international centers.
  214. We completed a pilot data envy data export and generation
  215. of an annual report and we are now ready for
  216. a wider rollout.
  217. Okay, okay.
  218. But Brisbane and Melbourne actively contribute to icon.
  219. Sydney has not started using Cardio sellers yet.
  220. New Zealand has recently started using Cardio cell and has
  221. joined the corners of April 2021.
  222. We have contributions from the university hospital in Leicester, Children's
  223. Hospital in Kuala Lumpur in Hong kong in Singapore.
  224. You're walking through data sharing agreements as of April 2020
  225. money but they have committed to participating in icon.
  226. What is it that we are looking to collect an
  227. icon? It's all fairly straightforward.
  228. basic demographic.
  229. There are like age, gender, primary diagnosis and country, right?
  230. Primary and secondary procedure.
  231. The type of patch the size of the patch, whether
  232. it's cardio, sailor, Carlos, Phil neal.
  233. How many patches, what locations the patches were used.
  234. That whether or not the patient at any re operation
  235. related to call yourself during the first admission from a
  236. follower perspective, what people really like as if the patient
  237. at any re intervention, be it an interventional procedure of
  238. a surgical procedure related to carry yourself whether there are
  239. existing problems at the site of uh Gardasil implantation that
  240. haven't yet met criteria for intervention.
  241. What imaging modalities have been used to diagnose these problems
  242. and any details around the explanation.
  243. Yeah, yeah.
  244. If you're planning to use card yourself, you would strongly
  245. encourage you to join a car and set up a
  246. system for prospective data entry.
  247. We have endeavored to make the process as straightforward as
  248. possible. All you have to do is Union Canal johnson,
  249. she's the data manager and request a startup back.
  250. She sends you out a couple of funds to complete.
  251. And most of this information relates who is actually going
  252. to be using cardio self, not just people that are
  253. going to need a password for access to the database
  254. once you complete the forms and she receives them, she
  255. will grant you access to a dropbox folder which contains
  256. all the registry agreements.
  257. It contains all the ethics approval documents that contains the
  258. actual study protocol, which we needed to submit for ethics
  259. approval. It has all the steering committee minutes, it has
  260. all the papers that have been published, not only about
  261. cardio, sorry, not only for my unit, but from every
  262. other uh, center in the world.
  263. Right, Okay.
  264. And then red cap contacts you directly with the user
  265. name and you set your own password.
  266. And that's really all you need to begin entering that
  267. up. You will simply have to go to the red
  268. cap, the red cap on site via the you are
  269. a search field in any browser, select your project and
  270. your username and password.
  271. And we can entering data.
  272. Now we strongly suggest that you get local ethics approval
  273. for I call and in this respect, if you send
  274. us the forms and give us the names of any
  275. individuals uh in your research governance office, we can help
  276. you with completing and submitting them.
  277. Okay. Okay.
  278. Yeah. We haven't caught their as yet because a considerable
  279. amount of manpower is necessary to maintain and monitor continuous
  280. online presence.
  281. And and for a start, I don't even have a
  282. facebook or to the regard, but hopefully we'll get there
  283. someday. If there's any information you need or if you
  284. think there's any way we can help you join and
  285. contribute to I call or you have, if you have
  286. any new ideas, please don't hesitate to get in touch.
  287. Emailing anyone of us is the same thing as emailing
  288. all of us.
  289. Mm Thank you very much for listening.
