If behavior does not improve, At-Risk Intervention Plan will need to be started.
3. Survey student
Someone with a good rapport with the student should conduct the "student interview" and the "Forced Choice Reinforcement Survey" with the student.

All cones are label with teachers name and room number
All students traveling to and from the restroom use the orange cones.
Student traveling to the office, nurse, library, or another teacher's room use grade level cones.
The purple-dyslexia and white-RTI cones are for pull outs.
CONE USE PROCEDURES
Sign out in the log book, writing their name and time of departure with destination.
Honor and respect the cone to and from their destination.
Go to their destination in a timely matter.
Return with the cone and sign back in with their return time.

2. STUDENT PLAN OF SUCCESS
students with behavior problems in multiple classes will begin a "Student Plan of Success" form.
4. Behavior Plan
As a team come up with ideas to manage behavior.
Conference should be held with the student, parent/guardian, all teachers, SPED/504 Coordinator to discuss At-Risk Plan.
