Annual Risk 2-3%

13-20% present as first manifestation

Typically 1-2yrs after Dx, active phase
Myasthenic Crisis
MAC, DO R2
"Grave muscle weakness"
Features:
Generalized weakness, proximal predominance

Neck, bulbar, facial mm weakness

Ptosis and diplopia MC

Dyspnea!!!!
CRISIS!!!!!
Myasthenic vs. Cholinergic
Precipitants:
Infection, surgery, pregnancy, child birth, med adjustments, spontaneously**
MGMT:
1) ICU

2) Supportive and IVIG

3) RT for NIF/VC
Where we fail:
not recognizing severity of respiratory failure, not treating underlying stressors
