CDSMP Graduates Online Community

An On-line Community has been developed and implemented by San Francisco DAAS and NCOA for graduates of the CDSMP, an evidence-based health promotion program created by Stanford University.

project timeline and goals
by Sept, 2013

timeline and performance measures
BTOP grant will end by Sept 2013.
New city fund has kicked in to continue the project with the successful bidder through a RFP.
NCOA is selected.

Performance Measures:
participants think that the online community is helpful;
participants want the community to continue;
participants establish a healthy life style such as practicing healthy exercises and eating well;
participants continue to live independently in the community.
support groups, forums, discussion groups
The community will be allowed to join or start any new support groups, forum topics, or discussion groups.
Monitors will watch out for risky questions, disapprove inappropriate language or topics.
Discussion leaders will try to lead in the beginning of the group, but then stay behind the scene.
outreach and marketing
Project is to be promoted through every means, including all related websites, announcement at conferences or workshops, local and state coalitions of the CDSMP program, PSA's, national events , and individual outreach activities.
The intent is to recruit participants from the United States, but eligible participants living in other countries are also considered.

the future?

This project is now one of the components of the new SF Connected Project from October, 2013.

The team will continue to monitor the growth and evaluate the effectiveness of this online community.

We have confidence that this project will support the evidence-based health promotion programs, which have been proved effective in disease prevention.

This is a project currently funded by San Francisco Department of Aging and Adult Services (DAAS) partnering with National Council on Aging (NCOA), initially with a grant from the Broadband Technology Opportunities (BTOP). Since October, 2013, it is funded by San Francisco City and County..
The online community supports the graduates of the Chronic Disease Self-Management Program (CDSMP), an evidence-based health promotion program created by Stanford University.
It provides support groups, a library of resources, Live chats with experts, and encourages
participants to continue practicing what has been learned from the CDSMP workshops.
