CHORIOCARCINOMA malignant trophoblastic aggressive cancer of the placenta

early hematogenous spread to the lungs

spectrum of Gestational Trophoblastic Disease (GTD)
Hydatidiform mole --> invasive mole --> choriocarcinoma

subset of germ cell tumors Etiology:
preceded by various conditions
-hyadiform mole (50%)
-spontaneous abortion (20%)
-ectopic pregnancy (2%)
-normal term pregnancy (20-30%)
rarely occurs at primary locations other than placenta

1:30,000 pregnancies (up to 1:2000 in high risk)

High Risk Pop.
-Asian and African descent
-Pregnancy before age 20 or after age 40 Signs/Symptoms:
increased beta-HCG

vaginal bleeding

shortness of breath


CXR with infiltrates Prognosis:
despite malignancy, very sensitive to chemotherapy
cure rate >90%
exception when metastasis to brain and/or liver

Gross - characterized by hemorrhagic and necrotic tissue within the uterus
