Warm-Up (Questions to help you prepare to write your goals)

1. Describe your current level of physical activity. How often do you participate in moderate to high intensity activity in an average week?

-My current level of physical activity is to where I am physically active at least 2 days a week. During those 2 days I am doing moderate activity, nothing intense basic cardio and upper body activities.

2. Discuss at least three physical activities you enjoy participating in or wish to participate in.


- I enjoy doing fall training for Tennis. I enjoy participating in my bowling team. I also love to swim when I can.

Goals
Physical Wellness Goal:
- In the next week of training for Tennis i will be more prepared physically and not slack off.

Emotional Wellness Goal:
-I will write things down or speak to someone of confidence to make sure stress takes over it wont affect me

-I will talk to everyone on my team more and interact to make close bonds.

Academic Wellness Goal:
- I will bring my focus back to HOPE and start completing work again.

4. What do you think will be the most challenging goal for you, and why?

- I think that my academic goal will be the most challenging for me because I have stopped doing my work and its always hard to get back on track with so much going on.

5. Describe any challenges or hesitations you have about your goals or the activity requirements for this course.
- I don’t think I have time due to so many extra curricular activities and trying to make senior requirements

6. Explain how fitness testing and test results can affect your overall fitness.


- Fitness testing and test results can affect my overall fitness by showing me that I need to work harder to get the results I desire.


7. No matter your living situation, it is up to you to make daily decisions that affect your well-being. What is your role and responsibility for maintaining and improving your health?

- I feel that my main role is to just be willing to try new things and get up and active. Also it is my responsibility to making being active apart of my daily routine because if not ill eventually be the one to blame.
