Sam was told by his physician Dr. Ambree that his kidneys where starting to fail and they would watch his laboratory results over the next few months. Sam went to the customer portal and starting researching kidney failure. While researching, he cross referenced the medications he was taking.  He found reports that showed that 2 medications he was taking are known to produce kidney failure when taken together. Sam emails Dr. Ambree his findings. Dr. Ambree reviewed the reports and took 
Sam off of one of the medications. The lab results got better and Sams kidneys are no longer in danger. 
Dr. Ambree brings up what Sam found at the next quality meeting. The providers review the reports and decide to take action. The Electronic Medical Record data base is searched for anyone in the practice who is also on the same medications. 3 are found and the medications are changed. The Electronic Medical Record now gives a warning if a provider tries to prescribe the two medications together. Sam is sent a gift card, a thank you note and is highlighted on the consumer portal as a contributing member of the American Community Health Team. 

Sams Story
Jan was at home with a sick three year old and a two week old baby in January. She makes an appointment online for 10am. At 10 am she Logs into the customer portal and turns on her webcam. Jan and her child have an appointment with their nurse practitioner without leaving the comfort of their home

Sarah has five children ages 16 through 5 who need school physicals. The 16 year old also needs a physical for Boy Scout Camp. Sarah schedules a home appointment the next evening. Dr. Anderson comes to their house and does all the physicals. He fills out the Camp form and emails the school forms before leaving the house.

Stan finds out he has pre diabetes during his annual physical. With the prompting of his provider he and his wife enroll in a health class. While in the class he learns about the latest research on chronic diseases conditions. He asks “why hasn’t any one told us this before” The class teaches them the simple changes they can make to improve their health. The class members make the changes together and support one another. At the end of the class Stan no longer has pre diabetes, he is off his blood pressure medicine, no longer has angina (chest pain), and feels 20 years younger. 

Now once a month he and his new friends meet at a local restaurant to eat a delicious healthy meal and plan community events that encourage healthy living.
