Each of the Department's grant applications or Requests for Proposals specifies the type of budget to submit. Final Expenditure Reports There are two types of final expenditure reports, the FS-10-F Short Form and the FS-10-F Long Form. Use the FS-10-F Short Form if the report is being submitted within report due dates. Fiscal Forms - Grants Finance Requests for Funds Use an FS-25 to request funds from an approved grant.  The amount of funds requested at any one time may only include actual expenditures to date plus, where allowed, anticipated expenditures for the next month. Budget Amendments Certain types of changes to a project or a budget require the grantee to request prior approval from the Department. Use the FS-10-A form to submit requests to the office responsible for the grant. B (4) Participation in a statewide evaluation program. For the purpose of this RFP, the required independent evaluator is a vendor (purchased services), NOT a partner. These expenses will not exceed 2.5% of the annual project budget; • and (5.a.) Those costs related to the maintenance of a statewide LPP project reporting data system shall not exceed 1% of the annual project budget. The above allowable expense should be itemized as services from a vendor and does not require a specific percentage from your allocation.
